"Exercise is good for your brain" is common enough advice that it's easy to tune out. The actual research behind it is more specific and more remarkable than the generic version suggests: aerobic exercise doesn't just correlate with better cognitive performance, it appears to physically increase the size of a brain structure directly responsible for memory.
The Study That Changed the Conversation
A randomized controlled trial published in the Proceedings of the National Academy of Sciences (PNAS) followed 120 older adults for one year, assigning them to either an aerobic exercise group (walking) or a stretching control group. The aerobic exercise group showed a 2.12% increase in left hippocampal volume and a 1.97% increase on the right. The control group, by contrast, showed hippocampal volume decline over the same year, which is the typical pattern of age-related volume loss in this brain region.
Why the Hippocampus Specifically Matters
The hippocampus is the brain structure most directly involved in forming new memories, and it's also one of the regions most vulnerable to age-related decline. A treatment that measurably grows it, rather than merely slowing its decline, is a meaningfully different and more encouraging finding than most cognitive health research produces.
How It Happens
The PNAS study also found that increased hippocampal volume was associated with higher serum levels of BDNF (brain-derived neurotrophic factor), a protein that supports neurogenesis, the growth of new neurons, in the dentate gyrus, a subregion of the hippocampus. Exercise appears to increase BDNF production, which in turn supports this structural growth. This is a genuinely different mechanism than most cognitive interventions, which tend to support existing function rather than grow the underlying structure.
How Much Exercise It Takes
The PNAS trial used a fairly modest protocol: 40 minutes of walking, three days a week. This isn't an extreme or elite-athlete level of activity, it's a realistic, sustainable amount that most people could reasonably build into a routine. Exercise also independently helps regulate cortisol, which connects directly to why chronic stress is so damaging to memory, and supports the sleep quality that memory consolidation depends on, meaning its cognitive benefits likely compound with these other systems rather than operating in isolation.
The Bottom Line
This is one of the more concrete, mechanistically clear findings in cognitive health research: moderate aerobic exercise, walking three times a week, increased hippocampal volume and improved memory in older adults over the course of a year, while a comparison group's hippocampus shrank over the same period. Few interventions in this space have evidence this direct.
